Complimentary Ticket Release
  Anyone receiving a complimentary ticket must complete the enclosed liability form. This also applies to employees, birthdays, shop passes and seniors. For those 17 and under, please include a parent's signature and their Drivers License number.

link is broken. can i get the pdf emailed to me or the new link

i have some passes from snow jam and might come up for my birthday so i thought id fill the form out ahead of time.
Would i need to fill this form out everytime i use a complimentary ticket?

Snow Jam tickets will not require this release though.  Best wishes.
